Welcome to Optimal Physical Therapy. We are looking forward to helping you meet your goals.

To help us get to know you and your needs, and to assist us in designing the right program for you, we have a detailed questionnaire we’d like you to complete – plus the usual new-patient documents to be filled out.

If you’d like, you can come early to your first appointment and complete them in the office. But you may find it easier to download them and print them out, and complete them at your convenience – then just bring them with you to your first appointment; whichever you find more comfortable.
